Details of Experience:

Anita, a 74-year-old female with a medical history significant for multiple sclerosis, was admitted to the hospital on November 11th, 2025, following multiple episodes of lower extremity weakness and recurrent falls. She presented with increasing difficulty ambulating and a growing fear of falling. Anita utilized a walker for mobility and reported that on September 21st, her legs gave out, causing her to fall backward onto her buttocks. Over the following weeks, she continued to experience progressive weakness in both legs, further limiting her independence. On the day of her hospital presentation, Anita experienced another fall while visiting her neurologist. While ambulating with her walker in the medical office building lobby, her legs gave out again, and she fell onto one knee. Due to her declining mobility and safety concerns, Anita was referred to our community through NYP Hudson Valley Hospital for comprehensive sub-acute rehabilitation services.

Anita was admitted to The Emerald Peek Rehabilitation and Nursing Center with diagnoses of generalized muscle weakness, difficulty walking, and a recent history of falls. Her condition had significantly impacted her daily functioning, and she required both physical and occupational therapy to restore strength, balance, and confidence in her mobility. From the moment Anita arrived in our community, she demonstrated quiet determination despite understandable apprehension about her fall history. Her individualized plan of care included Physical Therapy focused on lower extremity strengthening, endurance training, neuromotor re-education, gait training, balance training, and eventual stair training as tolerated. Occupational Therapy addresses functional mobility and activities of daily living (ADLs) to improve her independence and ensure a safe return home.

Anita’s early rehabilitation days were challenging, as even simple movements required significant effort due to her deconditioned state and underlying neurological condition. However, with consistent encouragement from her therapy team and support from the nursing and CNA staff, she began to show steady, meaningful progress. Anita approached each therapy session with focus and commitment, celebrating each small achievement along the way. Her therapists frequently noted her attentiveness and willingness to learn. She asked thoughtful questions, followed safety recommendations, and remained receptive to feedback. As her strength and endurance improved, so did her confidence. Over time, Anita progressed to ambulating more safely with her walker throughout the facility, marking an important milestone in her recovery.

By the later stages of her stay, Anita demonstrated notable improvements in balance, coordination, and overall functional mobility. She also became more socially engaged within the community, participating in conversations and activities—an encouraging reflection of both physical and emotional recovery. Throughout her time at The Emerald Peek, Anita expressed sincere gratitude to the interdisciplinary team that supported her journey. She formed meaningful connections with staff members who admired her perseverance and positive attitude despite ongoing challenges.

Anita was successfully discharged on December 13, 2025, two days before her birthday, with improved mobility, increased confidence, and a comprehensive discharge plan to support her continued safety and independence.
